The website has been created to help local healthcare professionals rapidly find the information they need. The tool bar above gives you two options:-

  • Support, Self Management and Healthy Lifestyles


  • Clinical pathways - you can browse by specialty to find the most appropriate pathways for your patients, it will also provide any relevant Choose and Book clinic information


  • The section on 'Services to Support Admission Avoidance has now been moved to 'Urgent Care' accessed from the Clinical Pathways menu

To browse these sections you may need to use the 'down' option to get to the bottom of the long list - especially in the clinical pathways section

There is also a search function which will probably be the fastest way to find what you are looking for.

Pathways and information is being added all the time - please use the RSS contact e mail function at the top if you can't find what you are looking for.
